When to use it
Use this low-stakes participation check for a community workshop. The example is a textile repair session for people aged 16 or over who have clothing or household textiles to repair. It separates a scope check from an application and a confirmed place.
This is distinct from a user research recruitment form: it checks concrete workshop scope and availability rather than collecting a participant profile for later selection. Do not reuse this sample as an identity, legal or medical eligibility decision.
How it works
Four answer-based Yes/No fields express the criteria:
- Is 16 or over checks that age is at least 16.
- Has a textile item checks for clothing or household textiles.
- Meets workshop criteria requires both of those answers to be Yes.
- Can join this session also checks that the person can attend.
People who can join continue to questions about their item and an email for the organiser. The confirmation explains that the organiser will check the repair and email to confirm a place. Suitable people who need a later date reach a separate page with an optional email for future dates. Everyone else reaches a useful alternative, with no contact questions: a family sewing class for younger makers or a qualified service for electrical repairs.
Skipped application questions are not required. There is no blocked Next button because every answer deserves a clear outcome. Device and campaign details never decide participation.
Make it your own
Replace the example date, workshop scope and age policy with your actual session details. In the Logic panel, change the named Yes/No facts rather than independently recreating the same criteria on every page. Keep the introductory explanation and alternative outcome in sync. See conditional logic and calculated fields.
Test ages 15 and 16, both eligible item types, an electrical item, no item and a future date. Change an earlier answer after entering application details: skipped contact and item answers should drop out of the submitted response. This does not reserve places or enforce capacity.
Put the answers to work
Read Can join this session with the item description before replying. Use the future-date email only for the requested workshop updates. All three outcomes work on Free.
Use the event registration form when no screening is needed, or the lead qualifier for service enquiries with budget and scope criteria.
